Help Me Grow - Community Health Worker (Full Time)

The Home Visitor will work with families and children ages 0 – 3 years of age. The Home Visitor must complete home visits, maintain files/documentation, report and records as required and meet all deadlines.

Education & Experience: High school diploma, Bachelor’s degree in Social Work, Human Services, Psychology, Early Childhood, or a related field (required); experience working with individuals in crisis and within low-income communities.

  • Skills & Abilities: Strong documentation, verbal, and written communication skills; ability to motivate others, establish rapport, and work independently with clear boundaries and focus.

  • Technical & Practical Requirements: Familiarity with community resources and social work practices; computer proficiency; valid driver’s license, personal transportation, and insurance.

Parents As Teachers - Community Health Worker (Full Time)

The PAT Parent Educator provides home visiting services to eligible families with children from prenatal stages up to kindergarten enrollment, focusing on parenting education and school readiness. While the program does not include clinical or therapeutic services, it supports families through education, screenings, and referrals to resources like childcare and early education programs. The primary goals are to enhance parenting skills, promote healthy child development, and support overall family well-being.

Education & Experience: High School Diploma (required) and experience working with children and individuals in crisis and within low-income or public housing communities.

  • Skills & Abilities: Strong documentation, verbal, and written communication skills; ability to motivate others, establish rapport, and work independently with clear boundaries and focus.

Youth Program - Youth Worker (Part Time)

The Part-time Group Worker, under the supervision of the Youth Coordinator, is responsible for developing and implementing a developmentally based, educational program for an assigned class of school children. The curriculum must meet the emotional, physical, cognitive, and social needs of both the individual child and the group.

Education & Experience: At least 1 year of experience working with youth, or a related field (required). B.A. in Education, BSW in Social Work, a related field, or working toward completion (preferred).

  • Community & Child Interaction: Experience in community-based settings with the ability to engage children in developmentally appropriate ways while being sensitive to individual needs.

  • Professional Growth & Communication: Motivated to grow in the fields of education and social work, with the ability to communicate respectfully and effectively with parents.

  • Team Collaboration: Willingness to work in a team, share and receive ideas, and remain flexible and open to compromise.

MomsFirst is a home visiting program that supports pregnant moms less than 39 weeks pregnant and women who have experienced an infant loss in the last six months prior to enrollment. Our goal is to aid in healthy pregnancies and birth outcomes so our babies will reach their first birthday. We provide education that addresses risk factors for infant mortality. We offer referrals for mental health, smoking and substance abuse. We also provide help with finding a doctor for mom and baby, answer questions about prenatal care, infant care, birth control, and family planning, and distribute bus tickets for transportation to medical appointments WIC and work. Mom and baby will stay in the MomsFirst program until the baby is 18 months.
